I understand that I voluntarily request and consent to participate in a yoga conditioning session and a retreat offered by Chinatsu Kambayashi, Studio Early Birds LLC.

I recognize that yoga and retreat can require physical exertion that can be strenuous.  As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated.  I am fully aware of the risks involved.

If I experience any pain or discomfort at a yoga and retreat, I will immediately communicate that experience with my instructor or organizer.  I understand that my role in this agreement is to listen to my body, adjust the posture and activity, and ask for support from the instructor and organizer.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in yoga sessions and retreats.

I represent and warrant that I am physically fit and have no medical conditions that would prevent me from participation in yoga conditioning sessions and any activities like hike and plunge.

I assume full responsibility for any injuries or damages, known or unknown, which I might incur as a result of participating in a yoga retreat.

I understand that yoga and a yoga retreat are not a substitute for medical treatment or medications, and it is recommended that I concurrently work with my Doctor or Primary Caregiver for any condition I may have.  I am advised that if I am sick, I should consult my Doctor.  I am aware that the instructor does not diagnose illness or disease and does not prescribe medication.  

I agree to irrevocably release and waive any claims that I have now or may have hereafter against Chinatsu Kambayashi, Studio Early Birds LLC.  

I have read and fully understand and agree to the above terms of this Liability Waiver Agreement.  I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability including death of any and all past, present, or future claims on behalf of myself, my family, my heirs, assigns, personal representatives/agents, and estate.
